Registration Form

Where: Mount Hood Golf Course, Melrose MA

When: Friday, July 11, 2014

Time: Sign in starts at 12:00----Shotgun Scramble at 1PM

Entry Fee: $135 per person (includes greens fees, cart, player gift and after golf meal)

Format: Four person scramble

Team Prizes: 1st, 2nd, and 3rd places

Individual Prize:  $5,000 putting contest, Longest drive and Closest to the Pin.

**Note: Mulligans will be available (2 for $10 cash), limit 2 per person and must be paid day of tournament.**
